By submitting this form, each volunteer hereby acknowledges and agrees that he/she is participating in this project at his/her own risk and holds Wesley Chapel Church and all its pastors, officers, members, and/or employees harmless from and against any and all claims, loss, damage, charge, or expense to which they or any of them may be put or subjected by reason of volunteer’s participation in this clean-up Beautification and Preservation Project. You further acknowledge that Wesley Chapel Church is not responsible for the acts of others participating in this project and the other volunteers may include, but not be limited to, interested individuals, members of civic groups, religious groups or community groups, and/or persons convicted of crimes and/or violations of County ordinance and ordered to provide community service.

Release and Waiver: Volunteer does hereby release and holds harmless Wesley Chapel Church from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from Volunteer's Activities.

Medical Treatment: Volunteer does hereby release and forever discharge Wesley Chapel Church from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the Volunteer's Activities.

Insurance: The Volunteer understands that Wesley Chapel Church does not carry or maintain health, medical, or disability insurance coverage for any volunteer. Each volunteer should obtain his/her own medical or health insurance coverage.

Assumption of the Risk: Volunteer hereby assumes the risk of injury or harm in the Activities and releases Wesley Chapel Church from all liability for injury, illness, death, or property damage resulting from the Activities.
Photographic Release: Volunteer does hereby grant and conveys unto Wesley Chapel Church all rights, title, and interest in any and all photographic images and videos made by Wesley Chapel Church during the Volunteer's Activities.

Other: Volunteer agrees that this Release is intended to be as broad as permitted by the laws of the State of Georgia and that it shall be governed by the laws of the State of Georgia.

By completing this form you agree that you have read this Waiver and Release of Liability thoroughly and fully understand and enter into it as a volunteer for this project. No one has made any representations, statements or inducements that change or modify anything written in this Waiver and Release of Liability.
